Essential Duties/Functions:
-Receive and relay all incoming clientele
-Provide intake and referral for incoming clientele
-Collect from and assist the clientele in obtaining the necessary documentation
-Identify clients needs, determine appropriate action, and erfer appropriate services
-Provide follow-up on referals made
-Prepare, maintain, and file reports, forms and records.
-Date entry into the H-HEAP database program and Shah client database system
